The Medical Social Worker assesses the psychosocial needs of hospice patients and collaborates with an interdisciplinary team to implement personalized care plans. They also provide counseling to patients and families while serving as a liaison to community resources for financial and legal support.
Candidates must hold a Master in Social Work (MSW) degree and have at least one year of experience in a healthcare setting. A valid driver's license, reliable transportation, and BLS/CPR certifications are required.
