Key Responsibilities
The Ambulatory Social Worker provides case management and social work services to patients and their families across inpatient and ambulatory settings. They are responsible for performing assessments, developing treatment plans, facilitating discharge planning, and providing crisis intervention to enhance patient social functioning.
Requirements
Candidates must hold a Licensed Bachelor Social Worker (LBSW) or Licensed Masters Social Worker (LMSW) credential from KSBSRB. Experience in customer service is required, while clinical or medical social work experience is preferred.
