Key Responsibilities

Provides psychosocial support, counseling, and case management to patients and families to address chronic or acute illnesses. Coordinates complex transition plans and collaborates with interdisciplinary teams to ensure quality care and prevent readmissions.

Requirements

Requires a Bachelor's degree in Social Work and a valid Licensed Social Worker credential from the MN Board of Social Work. Candidates should have at least 2 years of social work experience, preferably in an acute care setting.
