Key Responsibilities
Manage a social work caseload to identify residents' psychosocial and emotional needs while coordinating care plans. Support residents and families through admissions, transitions, and discharge decisions within an interdisciplinary team.
Requirements
Requires a bachelor's degree in social work or a related human services field and at least one year of supervised healthcare social work experience. A valid driver's license and strong interpersonal skills are also required.
