What You’ll Do
We are looking for a Social Worker to join our team within the After School Intensive Outpatient Program (IOP).

As a Social Worker, you will collaborate closely with psychiatric medical providers and therapists on the patient’s primary treatment team to ensure a cohesive and effective plan of care.
Responsibilities and Accountabilities
Assessment and Planning: Conduct thorough assessments, maintain the care plan, and lead groups to enhance patient understanding and engagement.
Patient and Family Engagement: Maintain open lines of communication with patients and their support systems to involve them in treatment plan decisions and foster a collaborative approach to care.
Ethical Advocacy: Deliver and advocate for patient-centered, culturally appropriate, and age-appropriate care, ensuring that all services align with ethical standards and best practices.
Your expertise will be instrumental in guiding patients on their journey to recovery.
What Makes You a Great Fit
- Master’s Degree in Social Work with licensure in the State of Minnesota required.
- Experience in healthcare setting preferred
- Experience using electronic health records preferred
- Proficient in Microsoft Office Products including Word and Outlook
- Able to effectively and efficiently communicate in written and verbal form
- Ability to smoothly and rapidly shift between dissimilar tasks
- Ability to communicate well both verbally and in writing
- Ability to react quickly and calmly as part of a team to handle volatile and potentially emergency situations
- Ability to make effective decisions while in stressful situations and circumstances
- Compensation for this role ranges from $26-$34 per hour depending on education and experience.
Key Responsibilities
The Social Worker will conduct patient assessments, develop care plans, and lead therapeutic groups for adolescents in an intensive outpatient program. They will also collaborate with medical providers and maintain active communication with patients and their support systems.
Requirements
Candidates must hold a Master’s Degree in Social Work and possess a valid licensure in the State of Minnesota. Experience in a healthcare setting and proficiency with electronic health records are preferred.
