Provides comprehensive case management, care coordination, and discharge planning for inpatient and outpatient settings to support wounded service members and veterans. Coordinates care across multiple providers and evaluates patient outcomes to ensure timely and appropriate service delivery.
Requires a minimum of an Associate degree in Nursing and three years of experience as a registered nurse within the last four years. Must hold an active, unrestricted RN license and a relevant clinical certification such as CCM, CDMS, or CRRN.
