Department: Population Health & Case Management

Reports To: Chief Quality Officer

Position Summary

The Clinical Pharmacist at Kintegra Health is a vital member of the healthcare team, dedicated to enhancing patient outcomes through specialized medication management and direct patient care.

The Clinical Pharmacist will leverage their deep understanding of medications to consult with patients, healthcare providers, and support staff to develop and implement comprehensive medication plans.

With a commitment to patient safety and continuous improvement, this role requires a strong ability to manage and monitor complex medication regimens, educate patients, and ensure accurate documentation.

This role is ideal for a clinical pharmacist who is passionate about making a direct impact on patient health and thrives in a collaborative, patient-centered environment.

Minimum Qualifications and Required Skills

  • A minimum of 3 years of related work experience in clinical pharmacy
  • Management and Leadership Experience
  • Understanding of quality and healthcare initiatives
  • Understanding of Case Management Requirements for Managed Care Contracts
  • Proficient in use of EMR’s (Electronic Medical Records) and Microsoft Word Suite
  • Excellent communication skills (written and verbal)

Education: Doctor of Pharmacy Degree or BS Pharmacy Degree

Licensure: Unrestricted North Carolina Pharmacist License required

Key Roles and Responsibilities

  • Provide one-on-one patient consultations to discuss medication regimens, address concerns, and adjust treatments as needed in collaboration with the provider.
  • Coordinate with physicians, nurses, and other healthcare professionals to optimize patient care plans and outcomes.
  • Review patient profiles and medical history to ensure safe and effective use of medications through medication reconciliation for transitions of care.
  • Develop, implement, and continually monitor individualized medication plans for patients.
  • Educate patients on medication usage, potential side effects, and overall recovery options.
  • Act as a resource to support the healthcare team’s patient education efforts.
  • Work closely with other healthcare providers to coordinate care and ensure seamless treatment processes.
  • Contribute to team discussions on patient progress and adjust plans as necessary.
  • Maintain accurate and comprehensive documentation of medication plans, adjustments, and patient progress.
  • Monitor for and report any adverse medication-related events to enhance patient safety and system improvements.
  • Utilize strong analytical and problem-solving skills to anticipate, identify, and mitigate potential medication-related issues.
  • Develop and implement strategies to prevent future adverse events, contributing to a culture of continuous improvement.
  • Attend population health team meetings that address whole person-centered care management efforts for patients.
  • Conduct chronic care management for Medicare patients.
  • Address medication adherence efforts for the population health department.
  • Collaborate with the manager of population health and case management to achieve organizational goals for value-based care.
  • Remain competent in pharmaceutical best practices.
  • Other Duties as assigned

Key Responsibilities

The Clinical Pharmacist will provide one-on-one patient consultations and collaborate with healthcare providers to develop and implement comprehensive medication plans. They are also responsible for medication reconciliation, patient education, and maintaining accurate documentation to ensure safe and effective care.

Requirements

Candidates must hold a Doctor of Pharmacy or BS Pharmacy degree and possess an unrestricted North Carolina Pharmacist license. A minimum of 3 years of related clinical pharmacy experience, along with management and leadership skills, is required.
