Join Meritus Health as a Clinic Nurse Practitioner - Mobile Health (Part-Time) in Hagerstown, Maryland.
Key Responsibilities
- Provides quality primary healthcare and wellness mobile health services to targeted populations.
- Collaborates with primary care providers, specialty providers, and other agencies to develop comprehensive care plans tailored to individual needs.
- Provides anticipatory guidance to individuals, families, groups, and communities aimed at promoting health and preventing or reducing the risk of health problems. Coordinates and facilitates community education and screening initiatives to enhance health outcomes.
- Identifies patient teaching needs and either directs to appropriate programs or relates information to the patient one-on-one. Provide patient education of disease process & management.
Requirements
- Education - Completion of certified APP program as a nurse practitioner or physician assistant. Masters in related field required.
- Experience - Minimum of one-year clinical experience required.
- Physician Assistant required. Certification in BLS (CPR) in compliance with policy required. Current and valid driver's license required.
- Knowledge/Skills/Abilities - Leadership, clinical management, program development, infection control and quality improvement.
