Provide proactive and preventive primary care to senior patients, including wellness visits and chronic condition management. Report to the Associate Medical Director while managing a Medicare Advantage patient panel.
Requires a Doctor of Medicine or Osteopathic Medicine degree and board certification/eligibility in Geriatrics, Family, or Internal Medicine. Must have a valid Florida license, DEA license, and at least two years of experience in senior value-based care.
