Key Responsibilities

Conduct comprehensive in-home health evaluations, including physical exams and medical history reviews, to bridge gaps in care. Build trusted relationships with patients to improve health outcomes and provide connections to necessary care.

Requirements

Requires an active, unrestricted license and board certification as a Nurse Practitioner or Physician Assistant. Candidates must be able to travel locally and work comfortably in diverse home environments.
