As a Home Health Physical Therapist, you’ll deliver one-on-one patient care while playing a critical leadership role in supervising and guiding Physical Therapist Assistants (PTAs). Your expertise ensures every patient receives safe, effective, and goal-driven care—every visit, every time.

Your Impact
- Perform comprehensive evaluations and establish clinical diagnoses
- Develop and implement individualized treatment plans focused on functional outcomes
- Directly supervise PTAs, ensuring treatments align with the plan of care and regulatory standards
- Provide ongoing guidance, mentorship, and clinical oversight to PTAs
- Conduct reassessments and supervisory visits to ensure patient progress and quality outcomes
- Maintain accurate, compliant documentation for both your care and PTA services
- Collaborate closely with patients, families, and the interdisciplinary team
- Ensure infection control and patient safety standards are consistently followed

What We’RE Looking For
- Licensed Physical Therapist (PT)
- Experience supervising PTAs (preferred, but not required)
- Strong leadership, communication, and clinical decision-making skills
- Passion for accountability, quality care, and team development
Requirements
- Successful completion of accredited physical therapy program and state licensure exam.
- Valid Physical Therapy license
- 1 year of home health experience preferred
- CPR certification
- Valid Driver’s license
- Valid Auto Insurance
- HomeCare HomeBase experience preferred
Physical Demands
- Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
- Must be able to lift, carry, pull, or push up to 100 lbs..
