Key Responsibilities
The Home Health Medical Social Worker assesses patients' social and emotional needs while coordinating community resources and support services. They collaborate with the interdisciplinary team to integrate these needs into the patient's Plan of Care and maintain accurate clinical documentation.
Requirements
Candidates must hold a Master's degree from an accredited school of social work and possess at least one year of recent experience in a home health or hospice setting. A valid driver's license and an insured personal vehicle are also required for this position.
