Job description
Join Avalon Health Care as a Social Worker – PRN, Home Health & Hospice in St. George, Utah, United States. Brighten Life Home Health and Hospice in St. George, UT is looking to hire an experienced Social Worker for PRN or as needed work.

Key Responsibilities
The Social Worker identifies and addresses the social, emotional, and psychological needs of residents while assisting with community care planning. Responsibilities include developing comprehensive social histories, completing MDS and care plans, and documenting progress notes.
Requirements
Candidates must hold a Master's degree in Social Work or a related field and possess an MSW or LCSW license. Previous healthcare experience is preferred, along with basic computer skills and the ability to pass background and drug screenings.
