Key Responsibilities

The Medical Social Worker provides psychosocial assessment, counseling, and care planning for hospice patients and their families within an inpatient unit. They also advocate for patient needs, facilitate communication among the interdisciplinary team, and assist with end-of-life planning and resource coordination.

Requirements

Candidates must hold a Bachelor’s degree in Social Work and a current Ohio LSW or LISW license. A minimum of one year of healthcare experience is required, along with strong communication and documentation skills.
