Key Responsibilities
The Medical Social Worker will perform initial psychosocial evaluations and develop interdisciplinary care plans for homecare patients. They will also provide supportive casework and consult with physicians and health team members to address the emotional and environmental factors affecting patient health.
Requirements
Candidates must hold a Master’s degree from an accredited School of Social Work and possess or be in the process of acquiring ACSW certification. At least two years of experience, including one year in a healthcare setting, is preferred along with current BLS certification.
