Job Location: LEBANON CENTER - LEBANON, PA, Position Type: Full Time, Salary Range: Undisclosed, Family First Health is seeking an independently licensed LCSW, LPC, or LMFT to join our Lebanon team as a Behavioral Health Consultant. This is not a traditional outpatient therapy role. As part of our integrated primary care team, you'll work alongside medical providers and other care team members to help patients address behavioral health needs that affect their overall health and well-being. Using an integrated primary care approach, you'll provide timely assessment, brief evidence-based interventions, care planning, consultation, and connection to specialty behavioral health services when longer-term or higher levels of care are needed. What You'll Do

  • Provide brief behavioral health assessment and evidence-based interventions within the primary care setting.
  • Work collaboratively with Primary Care Providers and other members of an interdisciplinary care team.
  • Develop patient-centered treatment and care plans.
  • Use screening and measurement tools to monitor patient progress and guide care.
  • Provide consultation and behavioral health education to patients and members of the care team.
  • Utilize approaches such as Motivational Interviewing to support meaningful behavioral change.
  • Assess behavioral health and safety concerns and coordinate appropriate follow-up care.
  • Connect patients with specialty behavioral health and community resources when appropriate.
  • Document clinical care and communicate with the treatment team through the electronic health record. What We're Looking For You must hold a current, unrestricted Pennsylvania license as an: LCSW, LPC, or LMFT We're especially interested in clinicians who enjoy interdisciplinary teamwork, brief intervention, whole-person care, and working in a fast-paced healthcare environment. Experience in integrated primary care, healthcare, hospital, community health, or another interdisciplinary clinical environment is preferred. Why Integrated Primary Care? You'll collaborate directly with medical providers, help patients address immediate behavioral health needs, and connect them with additional services when more intensive or longer-term treatment is appropriate.
