Key Responsibilities

The Physical Therapist I will provide evaluation, treatment, and follow-up services to patients referred by a physician. Responsibilities also include preparing and maintaining accurate patient records such as evaluations, progress notes, discharge summaries, and billing information.

Requirements

Candidates must be licensed or eligible for licensure as a Physical Therapist in the State of Illinois and must have graduated from an approved physical therapy program. Required certifications include CPR, and relevant experience in outpatient and orthopedic settings is necessary.
