Location: Facility 1: The Grand at Mohawk Valley – Ilion, NY

Job Summary

We are seeking a friendly and compassionate Cardiology Nurse Practitioner or Physician Assistant to join our team. As a Cardiology Nurse Practitioner/Physician Assistant, you will play a crucial role in providing comprehensive care to patients with heart-related conditions. This is an exciting opportunity to make a difference in the lives of patients while working alongside a dedicated team of healthcare professionals..

Responsibilities

Perform comprehensive patient assessments, including physical examinations, medical histories, and diagnostic tests

Collaborate with cardiologists to develop and implement treatment plans for patients with cardiovascular conditions

  • Administer medications, monitor patient responses, and adjust treatment plans as necessary
  • Conduct cardiac stress tests, electrocardiograms (EKGs), and other diagnostic procedures
  • Educate patients and their families about cardiovascular health, preventive measures, and the importance of medication adherence
  • Provide counseling and emotional support to patients and their families
  • Knowledge of CMS Regulations and NCQA HEDIS Guidelines
  • Document accurate medical records to ensure compliance with OSHA and Government regulations and perform duties in accordance with site quality and safety systems
  • Collaborate with the interdisciplinary team
  • Analyze and organize the care of critically complex patients

Requirements

  • Minimum 6 months of experience in long-term care or a skilled nursing facility
  • Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
  • DEA License
  • CPR BCLS/BLS certified
  • Must hold a Master's Degree in Nursing
  • Completed a degree from an accredited Nurse Practitioner program
  • Preferred experience in cardiology: training provided
  • Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided
  • Must have great organizational and time management skills.

Travel Between Facilities Required

Prolonged standing/walking. Occasional lifting up to 20 lbs. Carrying medical equipment. Lifting/moving patients.
