Physical Therapist – Home Health

You’ll assess patient needs, develop individualized treatment plans, and deliver therapy that improves mobility, reduces pain, builds strength, and restores function.
As a Physical Therapist, You Will
- Perform and document initial evaluations to identify functional limitations and establish appropriate treatment plans.
- Develop and implement individualized therapy programs to improve movement, restore physical function, relieve pain, and prevent further decline.
- Monitor patient progress and adjust treatment plans to achieve the best possible outcomes.
- Educate patients and caregivers on exercises, safe mobility, and strategies to support independence.
- Collaborate with patients, families, and healthcare team members to ensure continuity of care.
- Follow infection control practices and universal precautions during patient care.
- Complete accurate documentation within company-established timelines.
- Support agency policies, procedures, and commitment to quality care.
Requirements
- Graduation from an accredited physical therapy program and successful completion of the state licensure examination.
- Current, valid Physical Therapy license.
- One year of home health experience preferred.
- Current CPR certification.
- Valid driver’s license and auto insurance.
- HomeCare HomeBase experience preferred.
Physical Demands
- Ability to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
- Ability to lift, carry, pull, or push up to 100 pounds.
