What You’ll Do
- Perform and document comprehensive initial evaluations to identify patients’ needs and establish individualized treatment plans.
- Develop and deliver therapeutic programs to restore function, improve mobility, relieve pain, and prevent physical decline.
- Monitor patient progress and adjust treatment plans to support the best possible outcomes.
- Educate patients and families on exercises, safety, and strategies for maintaining independence.
- Collaborate with patients, families, and other healthcare professionals to ensure continuity of care.
- Follow infection control practices and universal precautions.
- Complete accurate documentation within company-established timelines.
- Uphold the agency’s philosophy, policies, and commitment to quality patient care.
Qualifications
- Graduate of an accredited physical therapy program with successful completion of the state licensure examination.
- Current, valid Physical Therapist license.
- Current CPR certification.
- Valid driver’s license and auto insurance.
- One year of home health experience preferred.
- HomeCare HomeBase experience preferred.
Physical Requirements
- Ability to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
- Ability to lift, carry, pull, or push up to 100 pounds.
