Key Responsibilities

The RN Case Manager delivers skilled nursing care directly in patients' homes and develops individualized care plans in collaboration with the interdisciplinary team. They are responsible for conducting assessments, providing patient education, and maintaining accurate clinical documentation in compliance with regulatory standards.

Requirements

Candidates must be a graduate of a state-approved nursing program and hold a valid RN license in the state of practice. At least one year of nursing experience with a similar patient population is required, along with a valid driver's license and auto insurance.
