Key Responsibilities
The Behavioral Health CM Liaison develops and implements behavioral health strategies to support member wellness and coordinates care across interdisciplinary teams. They serve as the primary point of contact for members, facilitating access to services and ensuring continuity of care through collaboration with providers and internal stakeholders.
Requirements
Candidates must hold an active Michigan social work license (LLBSW, LBSW, LLMSW, LMSW, or LCSW) and reside in Michigan. A minimum of 2 years of experience in an integrated care setting supporting behavioral health or substance use needs is required.
