Key Responsibilities

The Case Manager is responsible for utilization reviews, resource management, and developing safe, timely discharge plans for critically ill patients. They also facilitate multi-disciplinary team meetings and ensure proper reimbursement through coordination with payor sources.

Requirements

Candidates must hold a current clinical license as a Nurse, Respiratory Therapist, or Social Worker. Previous experience in an acute medical case management or inpatient hospital setting is required.
