Key Responsibilities

The Care Manager assesses and evaluates member needs to facilitate access to behavioral health resources and integrated care services. They serve as the primary point of contact, managing care plans and coordinating with interdisciplinary teams to improve health outcomes.

Requirements

Candidates must hold an active Illinois license as an LCSW, LMFT, or LCPC and possess at least 2 years of post-degree clinical experience in a behavioral health setting. The role requires the ability to travel for face-to-face assessments and a valid driver's license with proof of insurance.
