Telephonic Behavioral Health Care Manager

Key Responsibilities

The Care Manager assesses and evaluates member needs to facilitate appropriate resources and develop care plans for optimal wellness. They monitor patient progress and resolve barriers to care while maintaining compliance with organizational strategies and HIPAA regulations.

Requirements

Candidates must hold a Master's degree in a behavioral health field and a current valid social service license such as LMSW, LCSW, LPC, or LMHC. At least two years of experience in a behavioral health professional role is required, along with proficiency in Microsoft Office programs.
