The Community Support Specialist provides ongoing behavioral health assessments, develops treatment plans, and assists consumers in accessing community resources and benefits. They also provide crisis support, monitor medication compliance, and document all services in accordance with agency and regulatory standards.
Candidates must have a high school diploma and one to four years of experience in a social services or nonprofit environment. Proficiency in Microsoft Office, Electronic Medical Records, and the ability to work flexible hours including evenings and weekends are required.
