Key Responsibilities
The Case Manager is responsible for utilization reviews, resource management, and developing safe, timely discharge plans for critically ill patients. They also coordinate with payor sources to ensure proper reimbursement and facilitate multi-disciplinary team meetings.
Requirements
Candidates must hold a current clinical license as a Nurse, Respiratory Therapist, or Social Worker. Previous experience in an acute medical case management setting and familiarity with discharge planning are required.
