The Nurse Practitioner provides primary care to PACE participants by performing comprehensive physical assessments and developing individualized plans of care. They collaborate with the interdisciplinary team to manage chronic and acute conditions in both clinic and home settings.
Candidates must be a licensed Registered Nurse and a certified Nurse Practitioner with at least one year of experience working with the frail elderly population. A valid driver's license and the ability to participate in an on-call rotation are also required.
