Key Responsibilities
Providers conduct comprehensive in-home health assessments for older adults and at-risk individuals to review medical history, identify chronic conditions, and close care gaps. They collaborate with primary care physicians, family members, and other medical personnel to ensure coordinated care and follow-up.
Requirements
Candidates must hold a Master's degree and current Nurse Practitioner licensure with prescriptive authority. Board certification in a relevant specialty and at least one year of experience are required, along with the ability to travel extensively.
