The physician will provide comprehensive primary care to senior patients, managing complex and chronic conditions within a value-based care model. Responsibilities include conducting patient visits, coordinating care with specialists, and maintaining accurate clinical documentation.
Candidates must hold an MD or DO degree and be board certified or eligible in Family Medicine, Internal Medicine, or Geriatrics. An active, unrestricted medical license in South Carolina is required, along with proficiency in electronic health records.
