We are hiring a Nurse Practitioner for a Hospice position providing end-of-life care to patients in their homes in the New Kensington, PA area. This position is eligible for a $ 10,000 first year success bonus to reward the accomplishment of key milestones during your first year!

What You’ll Do

Provide compassionate, patient-centered hospice care, collaborating with an interdisciplinary team to enhance comfort, dignity, and quality of life for patients and families

Conduct comprehensive clinical assessments, manage complex symptoms and medications, and develop individualized plans of care that align with patient goals and end-of-life preferences

Educate and support patients, families, and caregivers through clinical decision-making and care planning while ensuring accurate, timely, and compliant EMR documentation through strong critical thinking and problem-solving skills

• Nurse Practitioner Qualifications
• Bachelor of Science (BSN) degree from an accredited School of Nursing
• Master of Science (MSN) degree from an accredited School of Nursing
• A minimum of five (5) years of clinical experience
• Hold, in good standing, a license as a Registered Nurse and Nurse Practitioner issued by the state in which you work
• Experience in a community based Home Health or Hospice/Palliative Care practice is strongly preferred
