Occupational Therapist (OT) – Per Diem - Home Health Visits | North Port and Port Charlotte, FL

Matrix Home Care is seeking a licensed Occupational Therapist (OT) for an immediate PRN opportunity providing home health therapy visits in the North Port and Port Charlotte areas.

Position Details

• Location: North Port and Port Charlotte, FL
• Position Type: PRN / Per Diem
• Pay Rate: $70 per visit
• Setting: Home Health
• Start: Immediate need

Responsibilities

• Provide occupational therapy services to patients in the home setting
• Evaluate patients’ functional abilities, needs, and goals
• Develop and follow individualized treatment plans based on patient needs
• Assist patients with improving daily living skills, safety, mobility, strength, coordination, and independence
• Educate patients and caregivers on therapy goals, home safety, adaptive techniques, and recommended exercises
• Monitor patient progress and communicate any changes or concerns to the care team
• Complete timely and accurate documentation in accordance with company and regulatory requirements

Collaborate with nurses, therapists, physicians, and other members of the care team to support quality patient outcomes

Requirements

• Active Occupational Therapist license in the State of Florida
• Home health experience preferred
• Strong clinical assessment, documentation, and communication skills
• Reliable transportation
• Ability to work independently in the field
• Professional, dependable, and patient-focused approach to care

Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.
