The Licensed Clinical Social Worker will serve as a member of the primary care clinic team to manage the behavioral health needs of the enrolled patient population. Responsibilities include providing patient-centered care, conducting functional assessments, and facilitating behavioral health consultations within the Patient Centered Medical Home model.
Candidates must hold a Master’s degree in Social Work from a CSWE-accredited program and possess an active, unrestricted clinical social work license. At least 12 months of experience in providing evidence-based individual counseling within a mental health or healthcare facility is required.
