Physical Therapist PT Home Health (PRN)

Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health Physical Therapist (PT) to work with us to provide exceptional and individualized care in south Pinellas county (including St. Petersburg, Clearwater, and surrounding areas).

Locations: Home health visits available in South Pinellas county (St. Petersburg, Pinellas Park, Seminole, Gulfport, South Pasadena, Clearwater, Largo). It is very flexible - you choose when & where you want to work!

If desired, visits may also be available in surrounding counties!

Schedule: PRN; 5-15+ home health visits per week. Required weekend/holiday rotations.

What You'll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled physical therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need

Active Florida PT License

• Minimum of 1 year experience as a licensed Physical Therapist (PT).
• Home health care experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday rotations
