Key Responsibilities

The Clinical Case Manager acts as a liaison between Healthmap, providers, and members to coordinate care and improve health outcomes for kidney management patients. They are responsible for managing an assigned caseload, conducting member outreach, and ensuring compliance with quality standards and documentation requirements.

Requirements

Candidates must hold an active, unrestricted RN license and possess at least 3 years of progressive experience in healthcare services or care management. A bachelor's degree is required, though 6+ years of RN experience with 3+ years in case management may be considered as an alternative.
