Key Responsibilities

The Care Navigator manages a caseload of kidney health patients, facilitating care coordination and creating member-centric care plans. They serve as a liaison between patients, physicians, and support networks while ensuring compliance with HIPAA and organizational policies.

Requirements

Candidates must hold an active, unrestricted RN license and a bachelor's degree, or have 6+ years of RN experience with 3+ years in case management. Experience in dialysis or transplant centers and knowledge of Medicare/Medicaid are preferred.
