RN Care Navigator (Eastern - Remote)

Key Responsibilities

The Care Navigator manages a caseload of kidney health patients, coordinating care and facilitating communication between patients, families, and healthcare providers. They are responsible for creating member-centric care plans, performing medication reconciliation, and educating patients on renal replacement therapy options.

Requirements

Candidates must hold an active, unrestricted RN license and a bachelor's degree, though 6+ years of RN experience with 3+ years in case management may be substituted. Experience in dialysis or transplant centers and knowledge of Medicare/Medicaid are preferred.
