RN Case Manager Full Time

Key Responsibilities

The RN Case Manager coordinates patient care, discharge planning, and integration of services across hospital departments and external organizations. They also perform psychosocial assessments, monitor utilization reviews, and ensure high-quality, safe patient care delivery.

Requirements

Candidates must hold a Bachelor's degree in a healthcare or social service field or be a Registered Nurse. Two years of hospital experience is preferred, along with current licensure as an RN or Social Worker.
