Key Responsibilities

The Physical Therapist will perform comprehensive patient assessments and provide therapeutic interventions in a home health setting. They are also responsible for coordinating care with the interdisciplinary team and participating in quality improvement programs.

Requirements

Candidates must hold a current Physical Therapist license in Colorado and have at least one year of professional experience. A BLS CPR certification is required, and familiarity with home health care and OASIS documentation is preferred.
