Physical Therapist - Home Health - PRN

Key Responsibilities

The Physical Therapist will provide evaluative, consultative, and treatment interventions to home health patients in their place of residence. They are responsible for collaborating with the interdisciplinary team to develop and manage patient care plans while maintaining accurate clinical documentation.

Requirements

Candidates must hold a valid professional license in the state of practice and be a graduate of an accredited Physical Therapy program. Prior home health experience is preferred for this role.
