Key Responsibilities
- Provide statutory social work support — delivering professional social work interventions and fulfilling legal duties under adult social care legislation.
- Promote independence and recovery — completing strengths‑based assessments and enabling individuals to achieve the best possible outcomes.
- Coordinate safe hospital discharges — working closely with health partners to ensure timely, appropriate discharge planning and community support.
