Join our Rehab as a Physical Therapist!
We are hiring a resident-centered Physical Therapist with Home Care experience to join our Certified Home Health team.
Our Physical Therapists provide a holistic approach, supporting the residents' physical, spiritual, emotional, and clinical needs. They are responsible for implementing residents’ personal care plans, focusing on residents’ strengths, preferences, and preferred routines, while providing a sense of comfort, companionship, and belonging.
All of our residents live within our award-winning campus (Home Care without the travel!), creating an environment where you can be part of their journey throughout their continuum of care.
Compensation: $48 - $52 per hour, commensurate with experience plus

Providing a holistic approach, supporting the residents' physical, spiritual, emotional, and clinical needs.
Implementing residents’ personal care plans, focusing on residents’ strengths, preferences, and preferred routines, while providing a sense of comfort, companionship, and belonging.
Working with the interdisciplinary clinical team to ensure the needs of our residents are being addressed throughout their care.
What You Will Need
- Must meet current Medicare and state requirements
- Must possess a valid and current Maryland State Physical Therapy license
- Experience and the ability to complete documentation to meet reimbursement and regulatory requirements (ie: OASIS, etc.)
- Previous Home Care Physical Therapy experience required
- Therapeutic experience in older adults across multiple diagnostic groups is preferred

The Physical Therapist provides holistic care to residents by implementing personalized care plans that address physical, emotional, and clinical needs. They work closely with an interdisciplinary team to ensure consistent support and quality of life for residents within the retirement community.
Requirements
Candidates must hold a valid Maryland State Physical Therapy license and meet all Medicare and state regulatory requirements. Previous experience in home care and therapeutic work with older adults is required, along with proficiency in clinical documentation.
