Key Responsibilities
The RN Case Manager coordinates the full continuum of patient care from pre-admission through post-discharge to ensure optimal health outcomes. They collaborate with interdisciplinary teams to manage discharge planning, insurance utilization, and patient resource needs.
Requirements
Candidates must have at least two years of professional clinical experience and hold a current license in the State of Utah as a Registered Nurse, Physical Therapist, Occupational Therapist, or Clinical Social Worker. The role requires strong analytical skills and the ability to work rotating shifts in a 24/7 healthcare environment.
