Job Description

Case Manager RN Summary: Responsible for the overall coordination of the patient's plan of care and the resources utilized for the patient's stay. Assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's health needs throughout the continuum of care.
Provides discharge planning through referrals and multidisciplinary planning and serves as a liaison between patient/family and the healthcare team to ensure necessary care is provided promptly and effectively. Relies on experience and judgment to plan and accomplish goals.
Performs a variety of tasks related to the coordination of care. A certain degree of creativity and latitude is required. Ensures that patients' unique needs are met in the most appropriate, timely, and cost-effective manner.
The ideal candidate must be highly detail-oriented and goal-oriented, with superior analytic, critical-thinking, and time-management skills. They must have a positive, empathetic personality and the ability to work effectively with others as part of a multidisciplinary team in a complex care environment. It is also vital that they have excellent communication and interpersonal skills and consistently display a professional attitude.

Requirements
- Active and unrestricted Registered Nurse license
- 2 years of Case Management experience (5 years preferred)
- Case Management Certification, including CCM, ACM or NACCM preferred.
- Ability to develop and maintain positive working relationships.
- LTAC/Acute Care Hospital experience strongly preferred.
- Strong computer skills preferred.
