Key Responsibilities
The Social Worker performs psychosocial assessments for patients and their families to develop outcomes for alleviating symptoms. They provide supportive counseling, psychoeducation, and casework interventions while consulting with the interdisciplinary group.
Requirements
The candidate must be an expert in social work capable of providing clinical psychosocial support in a hospice setting. Professionalism, collaboration, and a commitment to end-of-life care are essential.
