Clinical Social Worker - Mobile Mental Health

Key Responsibilities

The Social Worker will provide clinical and case management services in community-based settings, such as homes and schools, while developing, implementing, and updating individualized treatment plans collaboratively with clients and families. Responsibilities also include maintaining accurate clinical documentation, monitoring treatment goals quarterly, coordinating care with community partners, and providing crisis intervention when necessary.

Requirements

Candidates must possess a Master’s Degree in Social Work or a related clinical field, along with NYS licensure (LMSW/LCSW/LMHC). Essential requirements include the ability to travel to various community sites, access to a vehicle, a valid driver's license, and experience working with youth experiencing mental health needs.
