Key Responsibilities

Manage a caseload appropriate to your level of experience, including complex cases for experienced social workers.

Complete strengths‑based, person‑centred assessments and reviews under relevant legislation.

Carry out safeguarding enquiries, ensuring vulnerable adults are protected.

Support timely and safe hospital discharges using agreed pathways.

Requirements

Recognised Social Work qualification and SWE registration.

Experience completing assessments and reviews.

Experience in community settings and safeguarding.

Ability to complete Mental Capacity Act assessments.
