About the Role
RespectAdultDayHealthCare LLC in Winchester, MA is looking for a compassionate and dedicated Part-Time Social Worker with LCSW or LISW license to join our caring team. Responsibilities
- Conduct comprehensive psychosocial assessments for adult day health care participants
- Develop and implement individualized care plans in collaboration with interdisciplinary team members
- Provide counseling, crisis intervention, and emotional support to participants and their families
- Connect clients with community resources, social services, and support programs
- Monitor and document participant progress, needs, and changes in condition
- Advocate for participants' rights, dignity, and overall well-being
- Facilitate family meetings and coordinate discharge or transition planning as needed
Requirements
- Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program
- Current Massachusetts social work licensure (LCSW, LICSW, or LSW preferred)
- Experience working with elderly, disabled, or medically complex adult populations
- Strong knowledge of community resources, elder care services, and social service systems
- Excellent communication, documentation, and case management skills
- Compassionate, patient, and culturally sensitive approach to client care
- Ability to work collaboratively within a multidisciplinary healthcare team
