Physical Therapist (PT) – Home Health

Key Responsibilities
The Physical Therapist will conduct in-home evaluations and develop individualized treatment plans to improve patient mobility, strength, and balance. They are also responsible for educating patients and caregivers while maintaining accurate clinical documentation and coordinating care with other clinicians.
Requirements
