- Complete in-home physical therapy evaluations

Key Responsibilities
The Physical Therapist will conduct in-home evaluations and develop individualized treatment plans to improve patient mobility, strength, and balance. They are also responsible for educating patients and caregivers while maintaining accurate clinical documentation.
Requirements
